Important information for new clients

You must provide vaccination information for your dog before arriving for care. You can attach the certificate to this form, or email it later. We require a digital copy on file. Animals arriving without proof of current vaccinations cannot be accepted. Feeding instructions will be taken and confirmed upon arrival. Medication instructions (if relevant) will be taken and confirmed upon arrival. Please bring any medications in their regular labeled containers provided by your vet. Parker Pet Care provides all other items such as bedding, brushes, toys, etc.
