• All animals must have proof of current vaccinations on file with us, or submit a digital copy of these vaccinations prior to arrival.
• If you need care for your animal immediately, please call us rather than submitting this form.
• Submitting a reservation request does not guarantee a reservation. Your booking in only guaranteed once you receive a confirmation email from us.

Important information for new clients

You must provide vaccination information for your cat before arriving for care. You can attach them to this form, or email them later. We require a digital copy on file. Animals arriving without proof of current vaccinations cannot be accepted. Feeding instructions will be taken and confirmed upon arrival. Medication instructions (if relevant) will be taken and confirmed upon arrival. Please bring any medications in their regular labeled containers provided by your vet. Parker Pet Care can provide all essential care items (dishes, litter boxes, bedding, etc), however, you are welcome to bring small personal items for your cat if you wish. Please bring your cat in a carrier, for their safety – we will store the carrier for you while you are away.
